Registration for winter athletics at Ridge High School will open on October 2nd on Genesis.

Please use the RHS Winter 2023-24 Athletic Registration Form to register your child. It will become available on Oct. 2nd.

As part of the registration process, every student must upload the NJ State Athletic Pre-Participation Form and Health History Update Form by October 27th. The date of the examination must be after November 20, 2022. All pages of these forms must be combined together into one document and then uploaded to the Genesis registration form. (Please include the Vision Exam Form if your examiner has not documented vision exam results on the physical form.)

If you do not have access to a scanner, please use your cell phone and the NOTES app to combine pages into one document. Pages can easily be scanned and saved and then attached to the Genesis Form. Our district nurse and school physician will not be able to review the form if you so not submit it as one document.

If your child already has an Athletic Pre-Participation Physical Form on file from an examination after November 20, 2022 then you just need to submit a Health History Update Form.

Physicals must have a date of exam of November 20, 2022 or later.

Submissions are due October 27th

 

If you submit forms after October 27th, there are no guarantees that your child will be cleared for the start of the season. Once we receive your submission and deem it complete, it will be forwarded to a school physician for review. Parents can check Genesis, under your student's photo, to see if they are marked as being cleared for winter athletics.

 

Please note that all athletes at Ridge HS must complete IMPACT cognitive testing to be medically cleared to participate. Please see the Ridge Athletic webpages for specific information.
